Accessing Secured Forms

Directions:

Before accessing secured forms, please read the directions below.  You will need your username and password to submit a workorder.

1. Click on the Secure Forms link below
2. When prompted for your username and password, you must type your username in following manner:
   lfcisd\username.  You must have the lfcisd\ in front of your username for the system to recognize your username.  This is the same username as your district computer login username (usually first initial last name).

C.  After typing in your password click OK.
